Lots of partners are involved in transforming community mental health services in Oldham.
Key partners include many of our mental health staff, GPs and primary care colleagues, Oldham Council, Tameside, Oldham and Glossop Mind, Positive Steps, Age UK and Turning Point.
In Oldham there are five primary care networks. These are groups of GP practices working with mental and physical health services, social care, pharmacy, hospital and voluntary, community and social enterpise services.

Each of these primary care networks is supported by a dedicated team of mental health professionals. This includes:
- Mental health practitioner
- Mental health wellbeing practitioner
- Community navigator
- Peer support worker
Working across all five primary care networks is a diverse communities link worker.
They will work with your GP to provide the right mental health support for your needs. If you need more or different support than they can provide, they'll have a discussion with other professionals to agree who is best to help you.
This could include a pharmacist, social worker, employment, financial or housing support, voluntary, community and social enterprise (VCSE) colleagues, drug and alcohol, domestic abuse or loneliness support or another specialist.
Phased approach
We're taking a phased approach to transforming services in the borough. The south primary care network will be the first to go live with a living well team.
