Oldham Adult Learning Disability Service
Our team is made up of learning disability nurses, physiotherapists, speech and language therapists and psychologists, who all work together to provide a seamless service for adults from age 18.
We can support you if you have a mild, moderate or severe learning disability and are registered with an Oldham GP.
We deliver person centred, high quality, health-focused support to enable you to develop, maintain or restore a valued life in the community.
This is underpinned by the key principles of inclusion, rights, choice and independence.
Our core aim is to provide you with a highly specialist assessment and intervention plan which meets your needs. This will focus on health risks, behaviour, communication and psychological needs.
We will work in partnership with other agencies and families to achieve positive outcomes for you in the home, community and other relevant settings.
We accept referrals from service users, carers, social workers, GP's and other health professionals. All referrals will go through a screening process for eligibility to access the service.
