Heywood, Middleton and Rochdale treatment support team and Greater Manchester severe mental illness physical health team
We offer the following services to adults in the community:
Clozapine monitoring clinics
If you are prescribed clozapine medication, you will attend these clinics for regular monitoring of blood results and your physical health. You will also collect your clozapine prescription within these clinics.
Depot clinics
You will attend depot clinics on a regular basis, where you will be administered medication via an injection into your muscle.
Annual home visits
This service will support you if you are under the treatment team and not under active care coordination. The visits will incorporate environmental and mental health risk assessments.
Shared care clinic
We currently offer shared care for the response hub, if you are being started on antipsychotic and lithium medication. You will have your bloods, electrocardiogram (ECG) and physical health monitored in line with the shared care policy, along with being issued prescriptions prior to your GP taking over prescribing.
Severe mental illness team
People with severe and enduring mental illness are at a higher risk of poor physical health. We can support GPs to help you complete an annual physical health check, so you stay healthy.
