Stockport home intervention team is now part of the older people's community mental health team.
We provide care and support for those over 65 who live in Stockport and are experiencing a mental health crisis.
We can support you if you have diagnosed dementia, or if you’re experiencing symptoms of depression, anxiety or a severe and enduring mental health problem.
Our aim is to offer an alternative to hospital admission where safe to do so, by supporting you within your own home.
What support we provide
We offer short term intervention (typically 6 – 8 weeks), which can include assessment, medication management, work to reduce anxiety, along with other advice and information.
One of our priorities is to involve your carers and loved ones in any decisions. We also liaise with primary care, social service teams and the third sector as needed to ensure the best outcomes for you.

Our team is made up of highly experienced community psychiatric nurses, occupational therapists and support workers.
We are based at The Meadows and work closely with the other inpatient services there to ensure you are discharged as early as possible.
We accept referrals Monday to Friday - our experienced support workers are also on duty over the weekend and on bank holidays to support you once you’ve been referred.
