Stockport community mental health liaison service for older people
Our service is for individuals over the age of 65 experiencing mental health problems that are either diagnosed or un-diagnosed.
We offer services in a variety of settings from hospital, intermediate care, residential and nursing care and also within patients’ homes.
The aim of the service is to support the mental health care of older people and to ensure that their needs are being met appropriately by joint working with a variety of health and social care professionals.
Mental health practitioners and mental health support workers work to assess the needs of older people with mental health problems; who are not currently known to specialist mental health services. The service will develop care plans in order to meet the individuals needs and will support them regardless of location until the care plan is completed.
