There are three community mental health teams in Tameside and Glossop:

  • North community mental health team – covers Ashton and Stalybridge
  • South community mental health team – covers Hyde and Glossop
  • West community mental health team – covers Denton, Dukinfield and Audenshaw

Our teams support people with moderate to severe mental health problems that have a significant effect on their day-to-day life. This can include conditions like depression and bipolar disorder.

We accept new referrals for people between the ages of 16 and 64.

We’ve been working with our partners at Tameside Council to improve our services. We want to make sure you have the best possible care and experience.

In the past, our community mental health teams were made up of health and social care professionals.

The following changes were made in early 2025:

  • If you have mental health care needs, you will continue to be supported by mental health professionals working in our existing Tameside community mental health teams.
  • If you have social care needs due to your mental health, you will be supported by a new adult social care mental health team, provided by Tameside Council. This means if you find it difficult to manage life due to you mental health, so you might be able to get help under the Care Act.
  • If you have health and social care needs, you will be supported by both teams.

We’ll continue to work together to meet your needs and the needs of your carers.

Information about the changes has been shared with our current patients. You will have a key worker (previously called a care co-ordinator) who can offer any support you need.

Referrals for mental health support

Our community mental health teams accept referrals from:

  • Pennine Care mental health services
  • The Tameside neighbourhood mental health team
  • GPs
  • Health professionals
  • Social care professionals

You must be aged 16 to 64 and have a severe and long-term mental health condition.

Referrals for a social care assessment

If you want to refer someone for a social care assessment, please visit the adult social care mental health team webpage

We have occupational therapists working in our Tameside community mental health teams, with a dedicated therapist for each team (north, south and west).

Occupational therapy can support you when your mental health problems have a significant effect on how you manage what you want and need to do in your day-to-day life.

We’ll work with you to identify what's important to you and your recovery. We can support you to identify and work towards personal goals, overcome challenges and support you to live a meaningful life.

We can also help you to develop and strengthen the skills you need to function effectively in your environments. This can include adapting your home environment or providing equipment to manage  physical difficulties that impact on your daily life.

Your occupational therapy visits may take place in your home, local community or at Pennine Care headquarters in Ashton-under-Lyne.

We accept referrals from our community mental health team colleagues or consultant psychiatrists.

If you feel you would benefit from occupational therapy, please speak to your key worker (previously known as your care co-ordinator).
